Most apple varieties will remove easily when ripe. If you have to tug on them, they aren't ready to be picked. I put my hand under the fruit and lift upwards, if they don't come off with little effort I leave them be. Tugging too hard and removing part of the limb can reduce next year's fruit spurs.
edit...I should have added that ^^^ is for varieties with decent stems. Some varieties grow very near the limb and have very short stems. For those, you lift and gently twist.

Leaders on my trees are bent from biennial bearing. I can't reach that high to thin, so they are what they are, I guess.

Bent central leaders are okay, that helps to limit the ultimate height of a tree and keeps more fruit closer to the ground (and us humans). Broken leaders aren't okay. They'll result in a tree sending up lots of new vertical growth which means more pruning/training.
An easy way to thin is a backpack sprayer and some Sevin.

BV, Besides coming off easily you can check the drops for brown seeds. That's generally a sign they are ripe. Orchardists would know the Apple type and determine when to pick from that varieties general due date, look, feel, and taste. Always twist upward to remove the apple so the spur is not damaged for next year. how to upload photo to internet
